Procedure for Admission

Interested parents, school staff, or other involved persons should contact the school at 434-971-8636. Students can be admitted at any time during the year. The referring agency will be asked to submit a packet of information on the candidate including:

  • Application for admission
  • Most recent triennial evaluation and IEP
  • Report cards and academic transcripts
  • Psychological testing
  • Psychosocial and family history
  • FAPT meeting minutes
  • Records or recent reports from any involved agencies: e.g. juvenile court, family services, outpatient therapist, treatment facility
  • Discharge summaries and psychiatric hospital records

The treatment team will review the information and schedule an intake interview if the candidate meets admission requirements. This intake interview is provided free of charge, and is not a binding agreement. The parents/guardians, the candidate, and other persons as needed (e.g. a representative of the home school district, a caseworker, the current teacher) are invited to attend this meeting. A written description of Lafayette School services and school brochure are provided to participants and discussed during the intake.  Staff will communicate the admission decision in writing within two weeks of the intake meeting.  If the student is not offered admission, alternative placement options may be recommended. For some candidates, staff may request additional information prior to making a decision about admission.

 

Prior to the Start Date

Students and parents must complete all admission paperwork and activities prior to the student’s start date. The activities that must be completed prior to the start date include:

  1. Conduct an IEP meeting for placement at Lafayette School. Note: The school district representative (LEA, or local education agency) shall ensure that a representative of Lafayette School attend the IEP meeting for a placement at Lafayette School. If the representative cannot attend, the Director of Education or his/her designee will use other methods to ensure participation by Lafayette School, including conference telephone calls or faxing information.
  2. Completion of admission paperwork. Students may not start school until all paperwork is complete, including documentation of a physical examination within the past three years and a current immunization record. We require a physical examination form completed by a licensed health care provider. The office manager and admission staff will meet with and assist parents/guardians to complete the forms and submit the necessary material.
  3. Establishment of the legal guardian, and the role of various personnel and agencies in the care of the student.
  4. Scheduling the first family therapy session.
  5. Orientation to the program, including the following:

      • Philosophy and objectives
      • General policy
      • School calendar
      • Staff list and how to contact school personnel
      • Financial information
      • Emergency procedures
      • Parent involvement
      • Behavior management procedures
      • Visitation procedures
      • Scheduling transportation (with the LEA)
      • Completion of the contract for services (with the LEA)
